The Health Referral Scheme is delivered by Active Blackpool, designed to help inactive people with chronic mild to moderate medical conditions become and stay more physically active, whilst benefiting and improving their health.
Active Communities Health Coaching service is offered to help individuals create lasting change and improve overall health and wellbeing via a series of sessions exploring through a health coaching conversation to find out what matters most to an individual.
Sessions help to increase motivation, confidence, working together to establish realistic healthy behaviours, giving individuals a greater sense of control. Health Coaching can be an ideal addition to programmes such as Exercise Referral as it can increase confidence and resilience whilst improving an individuals' physical and metal health. In addition, it can also support any individuals who may not be eligible for programmes such as Exercise Referral or individuals who have multiple barriers which may be holding them back from taking part but would like support in improving their health and wellbeing.
Here are just some of the areas individuals have been supported in making sustainable change through Health Coaching support:
- More active lifestyle
- Reducing stress and low mood
- Managing health conditions
- Lowering weight or maintaining weight loss
Please ensure you have obtained patient consent prior to a Health Coaching referral. Further consent will be obtained within the 'Welcome to Health Coaching Sessions'. This session will offer a detailed overview and will explore a personalised Heath Coaching Commitment.
Patients must have at least one of the following to be referred into the service:
- Respiratory issues such as, Asthma or COPD
- Occupational lung diseases
- Other respiratory issues

Our Cardiac Phase 4 scheme, also known as long-term maintenance or community-based cardiac rehabilitation, is designed for individuals who have completed the initial phases of cardiac rehabilitation (typically Phases 1 through 3) and are looking to maintain their cardiovascular health over the long term. This phase focuses on sustaining the progress made during earlier rehab phases and promoting ongoing heart-healthy lifestyle habits. Engaging in a Cardiac Phase 4 scheme helps individuals build on the progress made during the initial rehabilitation phases and supports long-term heart health and overall well-being.Only referrals referred by the Cardiac Phase 3 team will be processed; all other cardiac referrals please refer via the Exercise Referral pathway.
