Adult Healthy Lifestyle Service commissioned by KCC Public Health.
 The One You Kent service is a holistic service embedded into the local Councils.
  • Tier 2 Group Weight Management - Referrals with a BMI of  = 30 kg/m2 and <40kg/m2 with no unmanaged co-morbidities.  Where the person has a comorbidity, the condition must be well managed (for example, Type 2 diabetes – no recent episodes of hyperglycaemia) and the person must confirm that their GP or other health professional has stated they are safe to be supported by a Tier 2 service.
  • BMI =27.5 kg/m2 for individuals from ethnic minority groups.
  • BMI between = 40 kg/m2 and <50kg/m2 with no co-morbidities.
  • The One You Lifestyle Improvement Service - 12 weeks of 1-2-1 support to encourage behaviour change on diet, physical activity and alcohol consumption.
